Damage Student Device Form/ Device Request
Please fill out information about device if it is damaged or you're having technical issue. Due to a high volume of technology on campus and through out the district being used, wait time for issues to be seen may vary. Please be patience for your call and DO NOT DROP OFF DEVICE IN FRONT OFFICE OR WITH TEACHER. Device will be needed on hand when called. Form can be filled out by teachers if the student need assistance. teachers should not be filling out form for student if they need a device and have not been checked out.
