This form is only for 7-12 VIRTUAL Students who have never changed their original password. All 7-12 hybrid students can see their school library media staff to assist with password changes.

*Students in grades K-6 do not need to have their passwords reset. Teachers and parents can retrieve K-6 student usernames and passwords in MISTARQ Student Profile.
