MOUTHGUARDS (Please refer to page 31 of the Information Booklet)

It is mandatory for all lacrosse players (every girl in Years 7 to 9 and team players in Year 10 and above) to wear mouthguards in training and competitive situations *

I will arrange to purchase a Custom-Fit mouthguard from OPRO via their home impression kit system.

I will arrange for my daughter to have her own mouthguard fitted by her own dentist.

My daughter is not in Years 7 to 9 or in a team and so does not require a mouthguard.

Ear Piercing/Jewellery (please refer to page 33 of the Information Booklet) *
