Employee COVID guidelines have been updated to align with CalOSHA ETS requirements. The updated requirements focus on keeping employees who are positive or experiencing multiple (3 or more) COVID-related symptoms isolated from the general population.

This form is to help determine if you should report to work based on your COVID status. At the end of this from you will be notified if you are cleared to return to work or if you need further assessment by the COVID Response Team.
