Parent "No Access" Campus Library Request

This form allows parents or guardians to restrict access to specific books for their child. Books listed on this form will be marked as "no access", meaning your child will not be permitted to check them out or read them from the school library.

Before completing this form, please follow these steps:

  1. Check your child’s school library catalog to confirm that the book is available in the library collection.

  2. If the book is available, complete this form to notify the school librarian and your child’s teacher that your child should not be allowed to check out or read the specified title.

Este formulario permite a los padres o tutores restringir el acceso a libros específicos para su hijo(a). Los libros indicados en este formulario se marcarán como "sin acceso", lo que significa que su hijo(a) no podrá sacarlos en préstamo ni leerlos en la biblioteca escolar.
