Interest Form Mason Band Aid & Chaperone Program
Thank you for your interest in volunteering to be a Chaperone or Band Aid! During Marching Band season, whether you are parent of a musician or guard student, we area all one team working together to support all of our students. Both Chaperones and Band Aids provide care for our students both during long events at Mason and when the band travels. Please complete this interest form so we can help find a good fit for everyone.

This year we are offering a series of training opportunities for our team responsible for Student Health and Wellness--Chaperones and Band Aids are on the front lines of that team. Sharing your availability will help us manage attendance and ensure everyone is trained and ready to support our students!

Overview:

Basic Required Training for Chaperones and Band Aids (select 1):
5/30—Chaperone/Band Aid Training 7pm MHS
7/18—Chaperone/Band Aid Training 6pm MHS

Required Mental Health Training for Chaperones and Band Aids::
7/18-- District QPR/Anxiety/Mental Health Training 7-8:30 MHS Harvard Room

For Band Aids: Required CPR/AED Certification (Select 1 session if not currently certified—if certified please share a copy of your certification or date of your training)

—5/31—Session 1 CPR/AED/FirstAid——FULL--WAIT LIST By Emailing Mindy
Session 1 at Mason Fire Department 6-10pm (they shared they usually complete in closer to 3 hours. There is a $20 cost for this course from the American Heart Association. Our Fire Department trainers will submit our class roster and we will receive certification cards electronically shortly after the course.
