Please complete this form ONLY if you are requesting a change in your student's instructional setting.

All Grades 7-8 (and 6th at SJ & YWLA) will return to face to face learning 4 days a week beginning Monday, October 5. Fridays will be asynchronous online for all students. MOA will remain in its current form until Friday, November 6.  This means that the window to change your student's "coding" to F2F or MOA has passed.  Your student must remain in the platform they are currently coded for 2nd 6 weeks. Parents must complete this form by Friday, October 2, in order to change your student's "coding status" for the 3rd 6 weeks. Please complete this form in order to inform our instructional planning for these next phases.

On Monday, November 9, on-campus instruction will expand to 5 days a week and MOA course offerings may change based upon participation.

Please note that you may not have the same teacher if you opt to switch between the online and on-campus school options. The Choice Form is specific for each MISD campus and can be found on each school's website.  Please complete the Choice Form for each student in grades 7-12. A form is not needed for elementary students.
