Application for Temporary Library Card
If you are not registered for a Heyworth Public Library District card and live in the district, you may sign up for a card. Your temporary card will be good for 1 month after the Library reopens. You will be emailed a library card number and PIN so that you can use our digital library to read books and listen to audio-books. To receive your permanent card, you will need to complete the process by bringing two proofs of address, including a photo ID, into the library and filling out a paper application.
