RHS Transcript Request Form
Former students, (Alumni), please use this form to request your RHS School Transcript. 
Current students, please contact Guidance.
 
Form must be filled out completely. Transcript request may take up to 10 business days to complete. Transcripts will not be sent if recipient Name and address is not complete. You will receive an email once your request has been completed. Please fill out one form per recipient address.

Academic Records FERPA Release
The Family Education Rights and Privacy Act (FERPA) is a Federal Law designed to protect the privacy of a student's educational records. The Law applies to all schools that receive funds under an applicable program of the U.S. Department of Education. This act protects your personal information from being distributed to third parties. With limited exceptions, most schools must have a signed acknowledgement from you before personal information can be released to a third party (i.e. parent, employer, college/university).  For more information about FERPA please visit www.ed.gov/policy/gen/guid/fpco/ferpa/index.html). By completing this RHS Transcript Request form, I grant permission to RHS to release the transcript.

Please provide the information below to whom and where you would like the transcript sent. Please note, most educational institutions require that Guidance send an official school transcript directly to avoid tampering, please check with the institution on which delivery method is expected.

"Official School Transcripts" are sent directly from Guidance to an educational institution, they are signed, certified, and sealed. "Unofficial School Transcripts" are for personal use, and will not be accepted by an educational institution.
