By signing below, I acknowledge that participation in basketball activities, including tryouts, involves inherent risks of injury. I agree to release, waive, and hold harmless the basketball program organizers, coaches, volunteers, facility owners, and all affiliated parties from any liability, claims, demands, or actions arising out of or related to any loss, injury, damage, or illness that may occur during or as a result of my participation in the basketball tryouts.

I also understand and agree that the program is not responsible for any personal property lost, stolen, or damaged during the event.

I, the undersigned parent or legal guardian of the participant named above, have read and understand this waiver and release of liability. I give my full consent for my child to participate in the basketball tryouts and assume all risks associated with their participation. I also authorize emergency medical care if necessary.
