Teacher Data Survey - Identifying Reading Challenges  

Teachers:  Thank you for taking the time to participate in this brief survey.  The purpose of this survey is to find out if teachers are noticing signs that an older student (6th - 12th grade) may have a reading disability and what steps, if any, they take to address those challenges with the student, school administration, and parents.  In answering, please consider only those students who do not to your knowledge have a formal diagnosis of Dyslexia or another reading-related learning disorder except ADD.  Please limit your answers to students in 6th through 12 grades.
