Chickasha Public Library Student  Virtual Card Application
This card is for students under the age of 18. If you are 18 or older, please use the  Chickasha Public Library Adult Virtual Card Application.   Parent/Legal Guardian should fill out this form if they wish for their child or teen to have access to the Chickasha Public Library's virtual material. A virtual card provides access to all of the Library's virtual content, including the Oklahoma Virtual Library and Brainfuse Help now (online tutoring). Physical items CANNOT be checked out. If you already have a Chickasha Public Library card, you have access to the virtual content. You do not have to fill out this form.  Once we process your application, you will receive an email with your card number and instructions on how to get started. Please note the library only processes applications during library hours. You can call the Library at 405-222-6075 with any questions.
