Cruise Planning Form - Load Handling Equipment
This form must be completed (and supporting documentation provided) anytime load handling equipment (winches, wire, blocks, terminations, LARSs, etc.) is brought aboard R/V Falkor (too).  Forms are due five months prior to departure.

Please fill out one form for each component of your load handling system (i.e. fill out this form four times if you have a winch, a wire, a termination and an over-boarding block).

Working Load
Documentation from the manufacturer or a certified tester ("competent person", definition on the Load Handling policies page) is required for all pieces of load handling equipment.  Certificates must have been issued within five years of the end date of your cruise for winches, blocks, LARSs, terminations, etc.  Certificates or destructive test results for tension members (wires) must have been issued within two years of the end date of your cruise.
