Once you have completed this form, return the permission slip and NOTARIZED Medical Release Form to your band director. Extra Medical Release Forms can be found in the "Band Forms" section of our website. Remember, there is a Mandatory Day Camp on Saturday, May 9, from 9:00 AM to 5:00 PM at White Oak High School with a Parent Preview at 5:30 PM that evening. The $50 non-refundable deposit is due at this time as well.
