Current Northwest students who are under the age of 18 must have parent/guardian approval to release information to post-secondary institutions before your request can be processed. If parent/guardian has not previously granted this permission through the annual online registration verification process, there will be a delay in processing your request.

Note: This form is only required to request an official transcript. Please contact your school counselor if you only need an unofficial transcript to self-report your high school classes/grades.

This form is only available for current/recent students to request transcripts. Former Northwest students who graduated more than one year ago must request final transcripts from the LPS Student Services Department. These transcripts can only be requested by the student.
