NEST Learning Lab Request 2025-2026

Please complete the information below to schedule a student for the NEST Learning Lab for make-up work or make-up tests, assignment or test completion, extra help, tutoring requests, independent study, or extended work time. PLEASE NOTE: Make-up quizzes and tests can only be made up during Activity Period, Wednesday mornings 8:00 -8:55 am or M, Tu, Th, or F after school. Juniors and Seniors may also complete make-ups during Study Hall.

Students: It is your responsibility to complete this form BEFORE you come to the NEST Learning Lab to make-up a quiz or test. If the time you have scheduled is not available we will contact you. Otherwise, plan to to be in the Chapel at the time that you requested on this form.

Parents and Students, if you are requesting tutoring services, please complete the information below and we will contact you to set up a tutoring schedule. For the DATE section, please just choose the first available date and we will make adjustments as needed.

Teachers, please send test materials to the NEST, or you can email us and we will come pick-up materials. If your test is online, please email us the access information.

B) Be̳f̳o̳r̳e̳ ̳s̳c̳h̳o̳o̳l̳ ̳8:00 ̳a̳m̳ ̳-̳ ̳8̳:̳5̳5̳ ̳a̳m̳.̳ ̳o̳n̳ ̳W̳e̳d̳n̳e̳s̳d̳a̳y̳: QUIZ or TEST Make-ups.

F) Study Hall (Juniors and Seniors only): The teacher will inform student what they need to work on or complete.

G) Test or Quiz Make-up Special Request: You MUST email kclanton@silverdaleba.org NO LATER than 3:30 pm the day before to schedule a time that is not listed above.

H) Tutoring needed. (NEST staff will contact parents and arrange tutoring. (Student will be responsible for bringing $10 payment per session to the tutor)
