We have generated this form to help us pre-fill the required Contact Tracing form that is required for Mass.       We ask that you please fill out the following form and answer the following questions.

In light of ongoing Covid cases in the Community, we will utilize the sign up sheet to maintain Contact Tracing as the need arises. At this time, we are open to full capacity, but we ask everyone to be mindful of their neighbor. Please do not crowd them and respect Social Distancing where necessary. Masks are not required, but we do have some available in the foyer if you desire.  

We ask that our Parish Community continues to be vigilant and if you or a family member are feeling unwell, we ask that you remain at home.

 Thank you for taking time to fill out this form - it is not mandatory to attend Mass, however, it does help us to expedite the process. If you haven't submitted this form, if the need arises, you will be required to provide the information upon arrival at the Church.  

Please submit your form prior to Friday at noon. Any submissions received after this time will not be included and a contact tracing form will be required when you arrive at Mass.
