Carefully read the policies and guidelines below and agree to each point. You are required to accept and comply with these provisions in order to use the space, tools and materials. Thank you. *

Prior to using the Fab Lab, Patrons (BCL Fab Lab Users) must (a) complete a Patron’s Agreement form; (b) verify that he/she is a patron of Boundary County library with staff; (c) follow safety rules: always wear closed toed shoes, do not wear loose fitting clothing or jewelry, always wear safety glasses (provided), no food or drink in the Fab lab; and (d) watch prerequisite videos for each machine he/she uses. *

Patrons must have an appointment and check-in with staff each time they use the BCL Fab Lab. Please call ahead of time if you know you cannot attend appointment. BCL reserves the right to cancel appointments if patrons show up 10 minutes late or more. *

Patrons must participate in mandatory safety trainings in order to use all machines; Patrons may not use these tools until in-person “certification” training has been complete. When using a tool that does not require certification ie. hand tools, the Patron is certifying that he/she is capable of using that item in a safe and proper manner. BCL Fab Lab staff will make available upon request materials such as manuals. *

Patrons under age 16 MUST be accompanied by a parent or caregiver to use any machines or tools, as the lab contains potentially dangerous tools. Children (or anyone) whose behavior is deemed unsafe will not be permitted access to the BCL Fab Lab. Fab Lab staff reserves the right to deny access at their discretion. Children 6 years old and under are not allowed in the Fab Lab.  *

The BCL provides the Patron with access to safety supplies in the Lab, including safety goggles, masks, gloves, First Aid Kits, and fire extinguishers. *

The Patron agrees that the BCL is not responsible for any manufacturing or other defects in the quality of workmanship or materials inherent in any of the tools or equipment. *

BCL FAB Lab Items used in the Lab are to be returned in the same condition as they were issued, barring normal wear and tear. The Patron agrees to pay for the loss or damage of any BCL FAB Lab items and further agrees to accept the Library staff’s assessment of fair restitution for damage, dirtiness, delinquency and/or loss of items in part or total. Failure to reimburse for loss will result in Patron being barred from using the Fab Lab in the future. *

 The Patron agrees that if any tool or piece of equipment becomes unsafe, or in a state of disrepair, he/she will immediately discontinue use of the tool and notify the BCL Fab Lab staff. Patrons must immediately report any accident/incident that occurs on BCL Fab Lab premises to a staff member. *

The Patron agrees to take precautions to avoid causing unnecessary mess or damage in the BCL Fab Lab. The Patron agrees to clean up his/her workspace in the BCL Fab Lab following use. The Patron agrees to inform the Lab staff member in the event that they are unable to return a work surface, tool or equipment to its original state. *

The Patron acknowledges that the BCL is only able to provide consumable materials on a limited basis. The Patron agrees to avoid wasting consumable supplies and materials. Items used in the Lab must be returned by the Patron to their original location.   *

Use of the BCL Fab Lab is intended for discovery, learning, and prototyping purposes. Production of goods to be sold for profit is contrary to the library’s mission and is not allowed in the BCL Fab Lab. The patron agrees to produce no more than five (5) of the same item using a BCL 3-D printer. The production of dangerous items and weapons in the BCL Fab Lab is prohibited. *

The BCL staff reserves the right to restrict access to the Fab Lab as deemed necessary to preserve a safe, secure learning environment. *

I affirm that the information I have provided on the Patron Information page is current, true, and correct. I understand that this information may be subject to verification.  I do hereby for myself, on behalf of my heirs, successors, and assigns, in consideration of being permitted to use tools and equipment, waive any and all claims against the Boundary County Library (BCL) and any BCL or Fab Lab staff, volunteers, employers, or agents for any personal injury, illness, death, or liability resulting from or arising out of the carelessness, recklessness, negligence and/or fault of the Boundary County Library.  I hereby for myself, on behalf of my heirs, successors, and assigns, in consideration of being permitted to use tools and equipment, agree to release and indemnify and hold harmless and defend the Boundary County Library, their offices, agents, volunteers, Board of Trustees and employees from any and all liability, loss, claims, and demands, actions, or cause of action for the death or injury to any persons and for any property damage suffered or incurred by any person which arises or may arise or be occasioned in any way from the maker or possession of tools, technology, equipment or supplies use in the BCL Fab Lab. The parties intend each provision to be severable and separate and apart from one another. If a court of law construes that any part of this release is invalid, such construction shall not invalidate the remainder of this release.  The parties agree that any and all disputes resulting in litigation will be commenced, litigated, and adjudicated only in the County of Boundary, State of Idaho pursuant the laws of the State of Idaho.  I have read this release, have no questions about its meaning and voluntarily accept the terms of this release by signing my name below.   *
Any Patron under eighteen years of age must also obtain the following consent and release before using the BCL Fab Lab.  I am the parent/guardian of the students listed above. I consent to this as the parent /guardian of the named minor child and state that I have read the above release, have no questions about its meaning and voluntarily accept the terms of this release by signing my name below.
