Historic Plaque Order Form
Thank you for your interest in purchasing a historic plaque for your home. The purpose of the plaque is to designate your home as a Contributing Structure of the Fan Historic District in the National Register of Historic Places. Part of this process is to confirm the date your home was constructed.
We estimate it will take 6 -8 weeks for new plaques to be available. The cost for both a Fan District and a Monument Ave plaque will be the same price at $100.
