Thank you for participating in our nutrition survey. The purpose of this survey is to measure your level of food insecurity. Individual answers to this survey will remain anonymous. Please answer each question honestly and to the best of your ability. Your participation in this survey is voluntary and you can quit at anytime for any reason.
