Nashville PITTIE Foster Questionnaire
Before completing this application please make sure you meet, agree to and understand the following requirements:

*  ALL current animals MUST be spayed/neutered, up-to-date on all vaccinations and on monthly heartworm prevention (unless otherwise advised by a licensed veterinarian).
*  A home visit is required by a representative of Nashville PITTIE.
*  We do not recommend female to female dog foster situations, although we will consider on a case by case basis.
*  If you rent your home or own in a home owners association complex, verify with management that you are allowed to have a PIT BULL and not just any type of dog.  

Nashville PITTIE will provide all medical care, preventatives, supplies, and food for all dogs in our program.

The following requirements are mandatory for all foster families:
In order to be considered as a foster family today, you must:
*BE  21 YEARS OF AGE OR OLDER.

*Have consent to foster from all adults living in your household.

*Have consent from your landlord or the person owning the premises in which the animal will live.

*Understand that this application is the property of Nashville PITTIE and will be retained in its files.

*Understand that Nashville PITTIE has full authority to approve or deny your Foster application.

*Understand that Nashville PITTIE reserves the right to verify all information submitted on this application, including veterinary information.

*Understand it is your responsibility to provide safe, warm, indoor housing for fostered animal(s).

*Understand that despite best efforts, some dogs/puppies may not survive due to physical or other health-related complications.

*Understand that I must spend as much time as possible socializing and playing with the foster animal(s).

*Understand that I must walk the dogs on a leash several times a day.

*Understand that I must closely monitor the condition of the fostered animal(s) and contact Nashville PITTIE immediately when problems are observed or when I have questions/concerns.

*UNDERSTAND THAT YOU MAY NOT TRANSFER OWNERSHIP OF ANY DOG FOSTERED FROM NASHVILLE PITTIE TO ANOTHER PARTY.  IF YOU MUST RELINQUISH FOSTER RESPONSIBILITIES OF ANY DOG THAT YOU ARE  FOSTERING FOR  NASHVILLE PITTIE, CONTACT A REPRESENTATIVE OF NASHVILLE PITTIE TO RETURN THE ANIMAL or DISCUSS ALTERNATIVES.

*Agree to comply with the Nashville PITTIE rules and regulations, codes of conduct,  instructions and mandates and understand that failure to comply may result in immediate termination as a Foster family.

*Understand that in handling animals there exists a risk of injury, infection, disease including physical harm caused by animals to me, my family or my pets. On behalf of myself, my heirs, personal representatives and executors, I hereby release, discharge, indemnify, and hold harmless Nashville PITTIE, its agents, servants and board from any and all claims, causes of actions, or demands and any nature of cause, including costs and attorney fees incurred or sustained by me in any way connected with my services to Nashville PITTIE but not limited to: animal bites, accidents, injuries, medical fees, property damage, and even death.

*Understand all services to be performed are at your own risk and Nashville PITTIE is not liable for any human or animal medical bills obtained as a result of fostering.

*I understand that the foster animal(s) are temporarily in my care and belong exclusively to Nashville PITTIE.
