2) Signature of the Emergency Medical Information Form and Parent Code of Conduct. After submitting this form, you will be presented with the link to sign this form.
After receiving this information, we will email you your registration payment information to complete your registration.
We will need these 2 items plus your payment for each swimmer you register.
