Maxwell - Parent/Guardian Input Form: Student Placement
Maxwell is committed to partnering with families to provide the best learning opportunities for all students. The student placement process elicits feedback from current grade level teachers, special area teachers, support teachers, administration, and families to ensure that placements meet the learning needs of each child.

The Parent /Guardian Input Form serves as the official mechanism in which parents/guardians are able to provide input for the student placement process. This year's form has again been adapted for online submission. All forms should be completed and submitted by Monday, May 6, 2019. If you are not able to access the online form at home, you may come to the school office to complete your submission.

Please note that consideration is given to all information supplied by parents/guardians.
