HMS/HSDM Student Group Registration Form - Fall 2021
All student groups must register once per semester: in the fall (late September) before the start of Q1 and in the spring (late March) before the start of Q3 . This provides the opportunity for student groups to update their student leadership twice throughout the year.

Student group registration is required in order to maintain an active status on the HMS/HSDM Student Group Directory and in order to submit student group budget requests each quarter. The Student Council Treasurers will use the official registration directory to determine which student groups are eligible to apply for funding.

New student groups that have never registered through the HMS/HSDM Student Council need to apply to be recognized by the HMS/HSDM Student Council. The link to the application and other relevant resources can be found in the New Student Group Application Section of this form.

What is your student group's general email? *
This email would be used to connect interested Harvard faculty and students to your student group. Some groups that do not have a general email typically put their student group leader's email as a contact.
Every student group at HMS/HSDM must register their group through this form in order to be considered active by the HMS/HSDM Student Council.

Student group registration will be used to determine:
1) Eligibility to apply for Student Council funding
2) Student group funding tiers
3) Continued maintenance of student group financial accounts and outside funding
4) Access to Student Council resources
5) Connect faculty and student to student groups
