I, the undersigned, hereby acknowledge and agree to the following:

  1. Voluntary Participation: I understand that participation in sports activities involves inherent risks, including but not limited to physical injury, and I voluntarily choose to participate in the Girlz Hoop LLC sports program.

  2. Assumption of Risk: I am fully aware of and accept all risks associated with participation in the activities provided by Girlz Hoop LLC, including but not limited to injury, loss, or damage to personal property.

  3. Release of Liability: To the fullest extent permitted by law, I agree to release, discharge, and hold harmless Girlz Hoop LLC, its officers, directors, employees, coaches, agents, and volunteers from any and all claims, demands, actions, or causes of action arising out of or related to any injury or damage sustained by the participant during the sports program.

  4. Medical Treatment: In the event of an emergency, I authorize Girlz Hoop LLC to seek medical treatment for the participant as deemed necessary. I understand that I am responsible for any medical expenses incurred as a result of such treatment.

  5. Compliance with Rules: I agree to ensure that the participant will comply with all rules and regulations set forth by Girlz Hoop LLC, including any rules related to conduct and safety.

  6. Photography and Media Release: I grant permission for Girlz Hoop LLC to use photographs, video recordings, and other media of the participant for promotional and marketing purposes.

Acknowledgment of Understanding: I have read and understand this waiver and release of liability. I acknowledge that I am signing this document voluntarily and with full knowledge of its contents. *
