SD City Professional Dev/Flex Activity Request Form
This form is to submit an activity into the Professional Development Calendar. Instructional Improvement (Flex) activities will also be entered into the District's Flex system.

Complete and submit form at least five (5) business days before the activity.

Turnaround time:
- Up to two (2) business days during off-peak periods
- Up to five (5) business days during semester start and end
