Please complete this Report Form if your Young Person, who is a Member of the 8th Wimbledon Scout Group, has developed COVID-19 related symptoms OR if they have had to go into isolation as a result of someone they are associated with developing COVID-19 symptoms (requiring them to self-isolate under Government Guidelines).
This Report Form may also be used as a Leader or Adult Support member who may have to isolate (in which case, class "your Person" as meaning you if you are self-reporting).
It should be completed ASAP.
