North Creek Absence Reporting Form
REPORTING STUDENT ABSENCES
Daily attendance at school is an important component of your child's education. When a student needs to be absent, arrive late, or leave early it is important that we hear from a parent/guardian.  Failure to report attendance related information to the school results in an unexcused Absence. 

**Please note that if we do not hear from a parent/guardian by 9:00 AM, we do call home to verify your child's safety.

If your child is going to be absent for multiple days, we encourage you to call the North Creek main office at (734) 433-2203 and select option 1 to report this.  Otherwise, please submit this form for each day your child will be absent.
