Release of Liability, Assumption of Risk, and Parental Consent
I, the undersigned parent or legal guardian of the child named in this registration form, hereby acknowledge and agree to the following terms as a condition of my child’s participation in the Fort Sask Beach Volleyball Camp (referred to as “the Camp”).
1. Acknowledgment of Risk
I understand that participation in volleyball and other physical activities carries inherent risks, including but not limited to sprains, fractures, concussions, dehydration, and in rare cases, more serious injury. I voluntarily accept and assume all risks associated with my child’s participation in the Camp, including any risks that may arise from the actions, negligence, or failure to act of the Camp organizers, coaches, volunteers, or other participants.
2. Medical Authorization
I confirm that my child is in good physical health and is medically cleared to participate in physical activities, including outdoor volleyball. I authorize the Camp staff or volunteers to administer basic first aid or seek emergency medical treatment if necessary. I understand that all reasonable efforts will be made to contact me in the event of a medical emergency.
3. Release of Liability
In consideration of my child’s participation in the Camp, I hereby release, discharge, and hold harmless the Camp, its organizers, coaches, volunteers, sponsors, affiliated entities, and property owners from any and all liability, claims, demands, or causes of action that may arise from my child’s participation in the Camp, whether caused by negligence or otherwise. This release includes personal injury, property damage, or loss sustained during or related to Camp activities.
4. Behavior and Conduct
I understand that my child is expected to follow all safety rules and demonstrate respectful conduct toward coaches, staff, and fellow participants. I acknowledge that failure to follow such rules may result in removal from the Camp without refund.
5. Refund Policy
I understand that all camp fees are non-refundable once registration is complete, except in cases of extraordinary circumstances (such as a documented medical issue or family emergency), which will be reviewed at the discretion of the Camp organizers. No refunds will be issued for missed days, participant withdrawal due to conduct, or cancellations made within 7 days of the Camp start date.
6. Weather Conditions
I understand the Camp takes place outdoors and may be impacted by weather conditions. Reasonable efforts will be made to hold sessions as scheduled or to provide alternative arrangements. Refunds will not be issued for weather-related disruptions.
7. Personal Belongings
I acknowledge that the Camp is not responsible for lost, stolen, or damaged personal items brought by participants.
8. Parent/Guardian Certification
By checking the box below and typing my full name in the signature section, I acknowledge that I have read, understood, and agreed to the above terms. I affirm that I am the legal parent or guardian of the child being registered and that all information provided is accurate and complete.
