    Team members wait to be told what to do instead of taking ownership over their role.
    It's difficult to delegate and get the results I want.
    I don't hold team members accountable for their results.
    Conflicts are often due to personality or style differences.
    Though I value teamwork, in reality,  results are achieved by individuals, not teams.
    I ignore, avoid, or poorly handle performance problems or giving constructive feedback
    Team members go "off the grid" and you don't hear back from them in a timely manner.
    It feels like I have to chase my team to get things done.
    Our hiring process takes too long and/or doesn't seem to find us the right candidates. 
    It takes too long for new team members to get up to speed and into high performance
    Meetings are often unproductive and/or take too long
    I'm doing more of "the work" in the business than I would like
    Key team members have quit (or I suspect they're thinking about it)
    The team, including me, are more likely to "fight fires" than focus on long-term priorities.
    Mistakes are being made or items are being handled incorrectly.
    Projects are completed over budget or behind schedule
