This form must be filled out before equipment can be rented or borrowed from Hanszen College. By filling out and submitting this form, the individual or organization using the equipment agrees to the following conditions:
- The individual or organization is liable for any damage that may occur during its usage of equipment
- The individual or organization will pay for replacement or repair of lost or damaged equipment.
