I agree that the following will be completed.
1. Sweep and mop floors (as required) wash tables, return tables and chairs to their designated storage space.
2. All trash will be taken to the Garbage Bins located outside (the key can be obtained from the onsite Facility Host or Custodian).
3. If the Kitchen is used - all food, equipment, and supplies need to be removed or disposed of, counters need to be washed and sanitized, sinks need to be cleaned, and dishwasher emptied of all items.
4. All equipment, chairs, tables, whiteboards, lecterns, etc. borrowed from other locations within the EIC Facility must be returned and organized in the space, or the exact location, where they were, or are to be stored.
5. Borrowed audio/video equipment, and cables, etc. must be returned to the EIC Office.
6. Current facility rental rates charged, as indicated on your "Scheduling Invoice" are due upon receipt and must be paid in advance of your schedule activity in order to confirm your booking. NO CREDIT.
7. Your facility rental time slot should include and take into consideration your set-up and clean-up time.
8. Any time over or extended beyond your prearranged time slot will be charged at the appropriate facility rental rate in 30 minute minimum increments. This must be arranged, discussed, and settled at the time of your activity.
9. A $30.00 "Administration Fee" will be applied for ANY Changes or Cancellations related to your booking.
10. Full Rentals Charges will apply to ANY & ALL Cancellations made 2 weeks or less before your scheduled event. "No Shows" will also be charged the full facility rental rate.
11. A $50.00 "Cleaning Fee" will be applied for failure to complete the stated cleaning requirements, as above.
12. Additional fees may be charged for cleaning, to compensate for the time required to return the facility space back to acceptable standards.
13. The expenses for ANY and ALL damages incurred will be charged back to the Program Coordinator in order for EIC to recover the costs for such repairs.
