1. Payment is in the form of reimbursement directly to employers.

2. Invoices must be submitted within 20 days past the date of the assessment.

3. Health navigators eligible to take the competency assessment are those who successfully completed a recognized training program (https://cdphe-health-navigator-prod.appspot.com/PublicTrainingPrograms).

    a. Completed a CDPHE-funded training program: Otero College, Patient Navigator Training Collaborative, Boomers Leading Change, or Metro State University at Denver.

4. Navigators must complete the following steps prior to taking the assessment:

    I. Complete the Colorado Health Navigator Registry Application (https://docs.google.com/forms/d/e/1FAIpQLSfl7DTP9DNdVd64AcsA97KpG7E_fp5fhsstXiBDkNu1X4s1hQ/viewform).

    II. Register for the assessment no later than 20 days prior to the event (https://www.eventbrite.com/o/center-for-advancing-professional-excellence-cape-17337456566).
