F45 South Buona Vista Membership Freeze Request Form
We are always happy to freeze your account when you are travelling or unable to come to the gym.
Here are a couple of things that you need to know about Freezes:
---> We cannot do indefinite or open ended Freezes - our system needs a start and end date to enter a Freeze
---> Freezes can be for as long or as short as you like (but they must have an end date - see above)
---> There is no retrospective Freezing (i.e. Freezes cannot be back dated)
---> There is a Freeze fee of $35 which will be charged to the credit card on your account. If the fee fails because the credit card is declined, the Freeze will not be processed.
---> We require a minimum 3 days notice.
---> You are unable to book into any classes (current or future) during the Freeze period

If any of these conditions are not met, the Freeze will not be processed.
