2017 Home and School Mother Seton Luncheon Reservation Form
Complete one reservation form per school. Schools with reservations of 10 will be seated together at one table.
There are 10 seats per table; however, a school does not have to fill a complete table.
Table assignment is based on first come first serve basis.
Reservation is not complete until payment is received.

Two Tables per school only.

Each Home and School Association will be responsible for paying for their school's Mother Seton Recipient and one guest.
Please include this amount in your payment and table count.
Cost is $35 per person.
