Mountain Day Permission Form 2024

The Mountain Day tradition continues!  Mountain Day is September 20 this year.  This is our online permission form. Please read and fill out the form below. (To learn more about TA's Mountain Day tradition, click here.)

If you have multiple students at TA please fill in this form separately for each student.

Thetford Academy is a non-profit educational institution. References to Thetford Academy (henceforth referred to as TA) include its trustees, employees, volunteers, participating organizations, agents and assigns.

I understand that: 

TA is not an agent of, and has no responsibility for any third party including any organization which may provide services including food, lodging, travel, or any equipment associated with this Mountain day.

Despite precautions and supervision by TA, accidents or injury may occur in the course of participating in this activity. I assume ALL risk associated with the above noted student’s participation in Mountain Day on September 20, 2024. 

I acknowledge that I will not seek to have Thetford Academy held liable in the event of an accident, injury, loss of property or any other circumstances or incidents occurring during or as a result of participation in the outing referenced above. I hereby release and agree to hold harmless TA from any claims arising out of participation in this event. 

I authorize TA to act on my behalf in any medical emergency.

Thetford Academy  expects all students to attend Mountain Day, and we understand some may not be able to. (Please note: No students will be allowed to remain on campus that day.) *
Medical Concerns:
If your student has a medical condition that may require emergency medicine on Mountain Day please write the medication that the student needs to have with them in this section. 

Make sure your child has their medication on Mt day.

Please make sure your child's doctor has sent the health office a doctor's order for this medication.
