To register for our OS Giken USA Contingency Awards Program, please complete and
submit the following information. Once submitted and recorded, no further action is
required unless there is a change in status of any of the information submitted.

This will send your registration form to OS Giken USA, Inc. to have on file.

Please make sure all names, addresses, vehicle information, and member information are spelled correctly. OS Giken USA, Inc. is not responsible for incorrect, different, changed or misspelled information entered. All inputted information must match your Event Registration information to earn contingency awards.

Logo decals will be sent by USPS after completion of the online registration process. The provided logo decals must be properly placed as required prior to the start of the race event. Please allow up to 10 business days for delivery.

Only one driver per vehicle per event can be registered and eligible to earn contingency awards (Club Racing). Drivers using multiple race vehicles per event must register each vehicle and class separately.

All Contingency Awards must be claimed no later than 30 days after the race event.
Requests received after this date will be denied.
