2020 DC STEM Fair Media/Medical Release Form for Parents/Guardians
The DC STEM Fair is operated by the DC STEM Network. The DC STEM Network is a program of STEM partner organizations and the DC Office of the State Superintendent of Education. The Network is a partnership across industry, community-based organizations, schools, and STEM-rich institutions that work to increase the quality of STEM education in the District of Columbia.
PHOTO RELEASE AUTHORIZATION: I understand that participants of the DC STEM Fair may be photographed, video-recorded, or taped for education or news reports, videos, web/internet reports, and/or other promotional materials for the DC STEM Network or DC STEM Fair partners and donors. I further give permission for my child’s image to be included in such promotional materials.
MEDICAL RELEASE AUTHORIZATION: Authorization is hereby given to DC STEM Fair to act in the place of the parents of the named student should any emergency action (e.g. medical, surgical treatment or hospitalization) be required. It is understood that the DC STEM Fair and hospital authorities will make a bona fide effort to contact the parents/guardians before acting upon this authorization.
