TIME OFF / VACATION REQUEST
Please complete this form to request time off or vacation. All time off / vacation request should be submitted at least 2 weeks in advance to make arrangements for your absence. All employees (both hourly & salary) are required to submit requests anytime they wish to be off. For questions or concerns, please see Keri or your Division Manager.


Time Off / Vacation Pay effective 1/1/2011
All employees (both hourly and salary) will now be required to submit time off request forms.
You have the option to request vacation time or unpaid time off. Simply submitting a request does not guarantee that your time off will be approved. In rare circumstances it may be required for you to work. If that is the case, you will be notified of the denial. All requests should be submitted at least a week in advance to make arrangements for your absence.

Vacation time for the year is received on your anniversary date. Total vacation amounts will reset each year at that time. There will be no carryover of vacation hours. If you don’t have any time available; your paystub will not reflect any vacation hours.
For Hourly Employees vacation time will be taken in hours. One week’s vacation equals 40 hours. Your paystub will show your remaining vacation balance available.
For Salaried Employees vacation time will be taken in halves. So if you get one week’s vacation, you get 9 halves. Mondays – Thursdays equal two halves per day. Friday equals one half. Your paystub will show your total available vacation balance in halves.
If an Employee begins their employment as temporary and is later switched to permanent, their eligible anniversary date for receiving vacation will be set retroactively to the 1st day of temporary employment.

Employees will be compensated for all *unused vacation time. Example: If you get two weeks’ vacation per year and only take one week, you will be compensated for the vacation time that you did not take. You will be automatically paid for any unused vacation time on the week following your anniversary date. A request is not needed.

Freedom Industries does not offer employees paid sick time/leave. If you are out sick and would like to be compensated for that time off then a request for vacation time needs to be submitted. Any time taken off without a time off request form submitted will be subject to nonpayment. The following is a schedule of how vacation is earned:

Full Time Employees: One (1) week after One (1) year
Two (2) weeks after Five (5) years

Superintendents, & Foremen: One (1) week after One (1) year
Two (2) weeks after Two (2) years
Three (3) weeks after Five (5) years

*with the exclusion of Termination of Employment.
