School Meal Form HMMS
If you have NOT filled this out online, please have your student return this form to the cafeteria manager AT THE SCHOOL YOU WISH TO PICK UP AT. This first week will be gathering information to know how many meals to prepare for each group and verifying enrollment. The goal is for a group to pick up meals the day after they attend during the first week. Below is a pick up schedule. Please fill out only ONE application per family.

Staggered Group for attending school *
Meals will be offered to students the days they attend school. Students are also eligible to receive an additional 4 breakfasts and 4 lunches for the days the student is not at school due to the staggered rotation. These meals are what would be available to students during a typical school year. Professional development and in-serivce days students will not be served.
