This is form is for the student attending VOICE to fill out. We would like you to fill this out on your own, so that we can get to know a little more about you before you come to the conference. Before you begin filling out the form, please help us do the two following things.

- Schedule an interview 安排會談時間
Each student will have a 30 minute online interview face-to-face with a teacher so that we can get to know you, verify your level of English, and answer any questions that you may have about the conference. Please schedule an appointment before you begin filling out the rest of the form.
每位學生應安排三十分鐘與老師線上面談，讓我們對你有近一步的認識，確認你的英語程度，並回答你對於營會有關的任何問題。請在你填完表格前，先安排會談的時間。
- Read the Character Commitment 閱讀品格委身
Our goal is for VOICE to be a positive, memorable experience for you, and in order to do that, we also need your cooperation and willingness to participate in the camp. Make sure you understand our expectations of you, and we will answer any questions that you have about the rules during our interview.
VOICE 營會的目的是希望為你帶來正面、值得回憶的時光，為了達到這個目標，希望你能配合我們一起參與。請確認你明白我們的期望，我們也會在會談時，回答一切有關規則的問題。
We know that the application is complicated and takes time to answer. We appreciate you taking the time to answer these questions, as knowing more about you will help us do a better job in preparing for the conference.
我們知道這個申請步驟有些複雜，需要花多一點的時間回覆，然而因著對你有一定的認識，可以幫助我們更好的預備這個營會。
