This application is for the Bazaar hosted by the Islamic Center of Hawthorne on FRIDAY MAY 12th.

Please fill out this application and submit it in a timely fashion. Ensure you have read and

agreed to all rules and regulations before signing.

I, the undersigned, agree to claim any and all responsibility for the items listed below to be
sold.

The Islamic Center of Hawthorne (ICH) claims no responsibility for any damaged, stolen, or
lost merchandise.

I certify that all merchandise sold complies with Islamic laws, as well as U.S. laws. I accept
complete responsibility for all merchandise that is sold.
Sellers can choose their spots beforehand, first come first serve.

Permit fee: $50.00 per table (Max 2 TABLES)

regardless of sales or not. A courtesy chair and only one table will be provided by ICH free of charge.

PAYMENT: Payments by cash or check must be made to the office. Payments can also be done digitally through Zelle or Venmo to (310) 920-1273.

* PAYMENT MUST BE MADE A DAY BEFORE THE BAZAAR.
