Troop 4 requires that all Scouts submit this form before starting a Merit Badge. The only exceptions are Merit Badges worked on or completed at the Troop's week of Summer Camp. Upon submission, this form notifies the Scoutmaster, Advancement Chair and other MB Counselors.
