This form gives Minnetonka High School permission to send your transcript to post-secondary institutions that you're applying to. You must fill this out for all schools you're applying to directly (using the institutional application) and via Common App. **This form is for current seniors only.**
Your transcript includes your name, DOB, all courses you've completed in grades 9-11, your weighted GPA, and courses in progress. We will send a midyear transcript to the colleges you enter on this form after first semester grades are in. A final transcript is sent to your attending college upon graduation in June.
Some other important notes:
- Please read the directions on each section carefully. If you skip a step or do not follow the instructions, there will be a delay in your materials being sent. We will not know to send your materials to colleges without this completed form.
- Please allow your counselor and the CCC at least three weeks to process this form during peak application season (now-November). Pay close attention to your college application deadlines to ensure your materials are sent on time.
- You will be able to apply to up to six colleges at a time on this form. If you apply to more than six, you will have to complete this form in its entirety again.
