Student Digital Equity Survey & Transportation Request Form 2025-2026
A Student Digital Equity Survey is required by the state for every student.

If your student will need bus transportation for the 2025–2026 school year, the Bus Transportation Form will appear after you complete the Digital Equity Survey. Please note that submitting a request does not guarantee transportation, as eligibility is based on district guidelines and availability.
