Exit Feedback 2025-26 Season

Dear Parents,

We recently received your request to withdraw your child from a class at Boston Rhythmic. 

As a reminder, written notice is required before the start of your final month of classes. We will discontinue your automatic tuition payments 30 days after we received this form from you. All outstanding tuition must be paid in full. We do not prorate tuition for the final month.

To confirm your withdrawal, please complete the form below. Your feedback will help us understand your reasons for leaving our program and guide us in improving our programs for our students and families.
