Emerging Leaders Mentor Program- Mentee Application
(Note: Must be a current member of AL!VE and maintain membership throughout the length of the program in order to participate)

Thank you for your interest in the AL!VE Mentor Program. This program is designed as an opportunity for volunteer engagement professionals to offer guidance, support and information to fellow professionals that want to sharpen competencies that will assist them in their own professional growth and development.

If interested in participating as a mentee in this program, please complete this application. Mentees will be matched with mentors that can best assist the mentee in the competencies they are most interested in developing.

NOTE: Mentors will receive a copy of this application so they can learn more about the mentees and who they may best serve.

AL!VE will make every effort to find a match for everyone. However, not all members completing the application process may be selected as there may be more mentees than mentors available. Those selected will be based on what matches are most compatible.

Mentors and applicants will attend orientation in order to find a suitable mentor match. Once you attend orientation, mentors and mentees will be notified of their match by the following week.

Mentee Readiness Checklist
Can you say Yes to all the following questions?

1. Are you willing to commit to a virtual meeting monthly with your mentor for six months as well as attend orientation, a midpoint check in, and final meeting with all participants?

2. Are you willing to commit to initiating contact with your mentor as needed and being responsive to your
mentor's calls or emails?

3. Are you committed to coming prepared to your mentor meetings with an agenda, discussion topics, follow
up from previous sessions, etc. to make the most out of the time shared together?

4. Are you ready to develop an open, honest, and transparent relationship and commit to whatever ideas or
recommendations for professional growth your mentor may have for you to accomplish the goals you set for
this program?

5. Are you willing to complete the program evaluation to provide your candid feedback about this program, its
successes and opportunities for enhancement?
