All people visiting and using the Vancouver Soaring Association's facilities & fleet are required to complete this online form at the start of each day of their visit.

VSA is required to collect this information in order to continue operations during the COVID-19 pandemic.

Submitted responses will be visible to the club's directors and instructors. Contact information will only be used for contact tracing purposes.
