Thank you for taking this survey. I am an herbalist currently based in Atlanta, Georgia. I am considering whether to begin offering herbalist concierge care services in 2021 on the telehealth platform Healthie.

Herbal concierge means that you are able to pay an affordable monthly membership fee and have direct access to interactive herbal care on the Healthie platform. Access is either through a computer or smartphone.

Healthie allows clients and herbalists to easily communicate via messaging, schedule appointments, keep track of paperwork, and have private online meetings. It even has a feature for clients to set up a progress journal, with the option to share it with me so I can provide encouragement and feedback as we work towards your goals.

What type of herbal help do you want the most? *

Which price point and structure most appeals to you? Assume that each includes one bottle of professional-grade herbal extracts for each appointment.

$35 per month just in case I need to make a quickie appointment! (Includes up to four 20 minute sessions per year. Additional 20 minute appointments are $25.)

$375 yearly membership for one 60 minute annual session and two 30 minute appointments if needed/desired. Additional 30 minute appointments are $25.)

$1250 for a three month herbal coaching package to work on a specific goal. The package includes a 45 minute appointment every other week for 12 weeks (six appointments).
