This application is for students enrolling in the EKG Technician Training Module under the Self-Pay option. Please read carefully before submitting your enrollment.

📌 Course Format

  • This course is self-paced and offered online only.

  • You will complete modules at your own pace, with support available from Hawaii Medical Training Center (HMTC) as needed.

  • In-person attendance is only required when you return to complete your practice EKG tracings after passing your exam.

💰 Tuition & Fees

  • Total Program Cost: $1,495

  • Enrollment Fee: $500 (non-refundable, included in tuition)

    • Covers registration, 1 certification exam attempt, and access to the online course.

  • A minimum payment of $500 is required to begin the program.

    Following form submission you will see a list of links for the payment choice matching the choices you indicated on this application. Please be sure to complete your payment so that you may complete your registration.

📩 Enrollment & Course Access

  • Your signed enrollment form and paid Enrollment Fee will serve as your official confirmation of enrollment.

  • Once your payment is processed, you will receive an email (within 4 hours during business days; weekends may take longer) with access to Module 1.

  • When you finish Module 1, you must request Module 2.
    ⚠️ Please note: Once Module 2 is released, no refunds will be issued.

📚 Course Completion & Certification

  • This course is fully self-paced. There is no minimum or maximum time limit to finish.

  • When you have completed all modules and are ready to test, email HMTC to schedule your certification exam.

  • After passing the exam, HMTC will arrange your hands-on EKG practice tracings with our volunteers.

IMPORTANT PAYMENT POLICY:

Once your application is reviewed and approved, you will receive an invoice for the total amount (minus any applicable discounts on tuition only — not the enrollment fee).

You will have 24 hours to submit your payment. If payment is not received within that timeframe, your application will be discarded, and you will be required to reapply for the program.

Please note: Seats are only reserved for students who pay at least the NONREFUNDABLE enrollment fee within the 24-hour window. No exceptions will be made.

Payment Schedule.

PAYMENT SCHEDULE POLICY:
For students who do not pay in full at the time of enrollment, a payment arrangement is available. Students may divide their remaining balance into up to 3 payments, with the understanding that the full balance must be paid before taking the final exam—no exceptions.

The first payment is due the Sunday before your first day of class.

All remaining payments will be due weekly on Sundays, by 11:59 PM HST.

A payment is considered late if not received by the due date and time.

Late payments will incur a $15 late fee.

Returned payments will incur a $25 return fee.

Students with unpaid invoices will not be permitted to attend class until the account is brought current. Failure to comply with this schedule may result in dismissal from the program.

Student Acknowledgment – EKG Technician Training Program

Refund policy:

If I choose to withdraw before requesting Module 2, I am entitled to a refund of all monies paid except the nonrefundable $500 enrollment fee.

Once Module 2 is released, no refund will be issued — no exceptions.

By submitting this signed enrollment form and paying the Enrollment Fee, you are officially confirming your enrollment in Hawaii Medical Training Center’s Certified EKG Technician Training Program.

Program Cost & Payment Breakdown

Total Program Cost: $1,495.00

Enrollment Fee: $500.00 (nonrefundable; includes registration, course access, and one certification exam attempt)

Remaining Balance: $995.00

Refund & Withdrawal Policy

All withdrawals must be submitted in writing or by email. Text messages or verbal withdrawals will not be accepted.

If a student withdraws before requesting Module 2, all monies paid will be refunded within 60 calendar days, minus the nonrefundable $500 registration fee.

Once Module 2 has been released, no refund will be issued — no exceptions.

Students dismissed from the program or who stop participation after Module 2 is released are still legally responsible for paying their remaining tuition balance in full.

Retake & Re-Enrollment Policy

Students who withdraw before completing the program may be eligible to re-enroll in a future course at a retake rate of $1,000, once their original balance is paid in full.

H.M.T.C. may apply the $500 Enrollment Fee toward a future enrollment if the student re-enrolls within 90 days of the original enrollment date.

Collections & Financial Liability

Students with unpaid balances at the end of the program, and without an approved payment arrangement, will be referred to a collections agency.

This may negatively affect your credit score and future employment opportunities, as many employers conduct background and credit checks.

IMPORTANT: You are legally responsible for any outstanding balance, regardless of program completion. Please communicate with the Director if you are experiencing financial difficulties — we are here to help.

Program Delivery & Scheduling

The EKG program is offered online and self-paced. There are no scheduled class meetings.

Upon payment, students will receive Module 1 within 4 business hours (weekend registrations may take longer).

Students must request Module 2 after completing Module 1.

After successful completion of all modules and the certification exam, students will be scheduled to complete 10 supervised EKG lead placements/tracings at HMTC.

H.M.T.C. reserves the right to adjust module release timelines or exam scheduling due to unforeseen circumstances.
